Questionnaire
Before we carry out an assessment, we ask that a
questionnaire is completed to give us valuable background information to aid
diagnosis. We have two questionnaires. One for parents to complete for their
child and one for adults to complete about themselves
These questionnaires
are produced in Microsft Word and best completed electronically in that format.
This can be achieved by clicking on the ‘edit’ icon if viewing from Internet
Explorer which may be displaying the ‘MS Word’ icon. You will then be able to
display, edit, print and email from word.
Please choose either parent or adult as appropriate
